The Evolution of Active Surveillance in Prostate Cancer

For many years, the diagnosis of prostate cancer has almost always led to treatment — typically surgery or radiation. However, as medical understanding has advanced, doctors now know that prostate cancer behaves very differently from one person to another. Some forms are slow-growing and may never cause harm, while others are aggressive and life-threatening. This discovery led to the concept of active surveillance, a careful monitoring approach designed to balance effective treatment with quality of life.

Understanding the Spectrum of Prostate Cancer

Prostate cancer exists on a wide spectrum. At one end are cancers so slow-growing that they are often found only incidentally during autopsy — meaning the men lived their lives without ever being affected by the disease. At the other end are aggressive cancers that spread quickly, causing pain and serious illness.

Recognizing this range was key to changing how doctors approached prostate cancer management. Early research — including studies using extensive biopsy and surgical data at Johns Hopkins — helped define which patients could be safely monitored rather than treated immediately.

When active surveillance was first proposed, it was considered a controversial idea. Many experts feared that “watching” prostate cancer might allow it to spread. To ensure patient safety, the earliest criteria were very strict — selecting only men with the most indolent (slow-growing) cancers. Over time, as data confirmed that these patients did well, the criteria were broadened to include some men with slightly higher-risk features.

How MRI Transformed Active Surveillance

One of the biggest advances in active surveillance has been the introduction of multiparametric MRI (mpMRI). Before mpMRI, imaging could not reliably identify prostate cancer, and biopsies often missed the most aggressive areas.

While mpMRI is not perfect, it significantly improves the ability to find and target the most suspicious areas. This gives doctors greater confidence that a patient’s biopsy results truly reflect the overall behavior of their cancer.

As a result, certain older criteria — such as the number of biopsy cores with cancer — have become less critical. Today, even if multiple biopsy samples show cancer, a patient may still be a good candidate for active surveillance if the cancer is low grade and lacks aggressive features such as intraductal carcinoma or large cribriform Gleason pattern 4.

However, mpMRI and biopsies can still miss hidden aggressive disease. For this reason, ongoing monitoring — including periodic imaging and repeat biopsies — remains essential for men on active surveillance.

The Role of Age, Comfort Level, and Personal Choice

Active surveillance is not the right choice for everyone, even if they meet the clinical criteria. Older patients, for example, may be ideal candidates because their cancer is unlikely to affect their overall lifespan.

For others, the emotional aspect plays a large role. Some men are uncomfortable knowing they have untreated cancer, even if it’s considered low risk. The anxiety of “living with cancer” can outweigh the reassurance of close monitoring. These men may choose definitive treatment for peace of mind — and that is a completely valid choice.

Molecular and Genetic Testing: Adding Another Layer of Insight

Modern molecular testing has added a new dimension to decision-making. These tests analyze the genetic and biological makeup of the tumor to predict how aggressive it may be.

For patients who otherwise qualify for active surveillance, favorable molecular test results can offer reassurance that surveillance is the right choice. Conversely, if molecular findings suggest a higher-risk cancer, doctors may recommend additional biopsies, imaging, or treatment to avoid underestimating the disease.

Similarly, imaging findings must align with the biopsy. If the biopsy suggests low-grade cancer, but mpMRI looks concerning, further testing is often warranted.

The Ongoing Evolution of Active Surveillance

Active surveillance has come a long way — from a radical concept to a well-established management strategy for prostate cancer. Today, it is a personalized approach that considers many factors:

  • Pathology results (grade and pattern of cancer)
  • MRI findings
  • Molecular and genetic testing
  • Age and overall health
  • Emotional comfort and preferences

As research continues and new tools emerge, the criteria for active surveillance will keep evolving — always with the goal of ensuring the best outcomes while avoiding unnecessary treatment.
